Enterobacter cloacae Endophthalmitis: A Case Report

Abstract
|
|
|
Purpose: Enterobacter cloacae are commensal gram-negative organisms of the gastrointestinal tract and are considered pathogenic only for patients with lowered resistance to infection. We experienced a case of endophthalmitis caused by the gram-negative bacillus, Enterobacter cloacae which has not yet been reported in Korea.
Methods: A 47-year-old male had been hurted his left eye by a foreign body flew from mower revealed 3 mm sized scleral laceration near the limbus and best-corrected visual acuity (BCVA) of 20/100. On the day, He underwent primary closure of scleral laceration. A day after primary closure of surgery, the visual acuity had dropped to hand movement. Ocular findings such as corneal edema, hypopyon with fibrinous pupillary membrane in the anterior chamber and multiple low reflective vitreous membrane shown by ultrasonographic scan were consistent with infectious endophthalmitis. We provided intravitreal vancomycin and ceftazidime injection and vitrectomy. Culture of vitreous aspirate isolated Enterobacter clocae 6 days after traumatic event. Despite every effort, visual outcome in his left eye was handmotion 2 months postoperatively.
Results: Clinicians should be aware of the devastating effects of endophthalmitis which is caused by Enterobacter clocae.
